[developers] generator efficiency: index accessibility filtering

dear all,

in march this year, i checked in a change to the LKB generator so as to
enable index accessibility filtering in non-packing mode too.  my change
accomplished that, but (to my embarassment) also disabled the filtering
in packing mode.  the result was a sometimes sharp degrade in generator
efficiency, specifically on longer sentences with multiple modifiers.

dan discovered the deficiency over the weekend and provided a patch.  i
have just committed his patch to the LKB, hence would recommend a `cvs
update' to those using the generator.
